  • 갓 태어난 신생아에게 인공영양(영아 포물라/포물라)을 일률적으로 먹일 수 없고 모유를 일률적으로 수유할 수 없다.
  • 만삭 신생아로 태어났는지, 미숙 신생아로 태어났는지, 또는 어떤 건강 문제를 가지고 태어났는지, 어떤 건강문제가 없이 태어났는지, 다른 건강 문제의 유무에 따라 태어난 후부터 갓 태어난 아기에게 인공영양을 먹이는 방법이 조금씩 다를 수 있다.
  • 신생아들에게 모유나 인공영양을 먹이는 것도 하나의 영양 공급 의술이고 식이 예술이다.
  • 만삭에 태어난 건강한 신생아들에게 인공영양을 다음과 같이 먹일 수 있다.

  • 갓 태어난 신생아가 대체적으로 건강해 보이면 생후 2∼4시간 경에 5% 경구용 포도당액이나 경구용 신생아용 식수, 또는 끓여 식힌 맹물 중 한 종류를 선택해서 2∼5cc 정도 시험 삼아 맨 처음 먹여본다.
  • 갓 태어난 신생아의 입, 입안, 식도, 또는 위나 소대장관에 어떤 선천성 기형이 있으면 이런 액체 음식물, 즉 순 유동 음식물을 먹은 후 곧바로 구토하는 것이 보통이다.
  • 시험 삼아 처음 먹인 5% 경구용 포도당액 등을 토하지도 않고, 아무 이상 없이 젖병 꼭지로 잘 빨아 꿀꺽꿀꺽 넘기면 그 후 한 두 시간 후에 15∼30㏄ 정도 인공영양은 처음 먹여 볼 수 있다.
  • 그 후 아무 이상이 없으면 생후 1주까지 1회 분 인공영양을 1일 6~10번 정도 먹인다.
  • 생후 1주에서 1개월까지 1회분을 1일 6~12번 먹이고 생후 1~3개월에서부터는 1일 6~8번 정도 1회분을 먹인다.
  • 1회분 인공영양을 3∼4시간마다 먹일 수 있다.
  • 1회분 인공영양의 용량도 일률적으로 정할 수는 없지만 대략 다음과 같다.

생후 20일 되는 신생아는 3~4시간 간격으로 하루 6~8번 1회분 90~120cc정도 조제분유를 먹으면 됩니다.

매번 인공영양을 먹일 때 마다 1회분 권장량에다 인공영양 30cc를 가감할 수 있습니다.

우유는

소젖에서 직접 짠 생우유,

그 생우유를 저온 살균처리해서 만든 전 우유(Whole milk),

전 우유의 지방분을 탈지해서 만든 탈지 우유,

수분을 뺀 가루 분유

그 분유를 이용해 조제한 조제분유(포물라등 여러 종류의 우유가 있는데 어떤 종류의 우유를 먹이시는지요?

아기가 아직도 정식 말로 의사를 정확히 표시 할 수 없으므로 아기가 울 때마다 조제분유를 주시는 것 같습니다.

  • 그래서 임신 중 뇌하수체 전엽에서 프로랙틴 호르몬이 분비되지 않는다.
  • 출산 후 바로 태반이 자궁 속에서 반출된다.
  • 태반이 자궁에서 배출된 후 프로랙틴 분비억제 호르몬이 더 이상 태반에서 분비되지 않는다.

  • 아기가 젖꼭지를 빨면 내강(Lumen) 내 유동 속에 괴여 있는 젖이 젖꼭지에 있는 젖구멍으로 분비된다. 또 뇌하수체 전엽에서 프로랙틴이 계속 분비되어 포생샘(Alveola gland/젖샘/유선/)에서 젖 생성을 촉진 시킨다. 젖샘 세포에서 젖이 계속 만들어지기 때문에 젖을 때를 맞춰 아기에게 먹일  수 있다.

분만 후 언제부터 모유 수유를 시작할까?
  • 갓 태어난 아기에게 언제부터 모유 수유를 시작하면 좋으냐고 묻는 산모들도 있다.
  • 산모 엄마와 갓 태어난 아기 모두 건강하다면 자연분만을 한 후 분만실 의자에 앉아서나 제왕절개수술 부만을 한 후 분만실 분만대 위에 누워 분만회복을 하면서 젖을 먹여도 된다.
  • 즉 산모의 건강상태, 갓 태어난 아기의 건강상태, 엄마와 갓 태어난 아기의 건강상태에 따라 첫 모유 수유를 시작할 수 있다.
  • 둘 다 건강하면 분만 후 바로 아기에 젖꼭지를 물려 젖을 빨아 먹게 할 수 있다. 자연 분만을 한 후 처음 젖을 줄 때, 한쪽 젖을 약 3~4분 간 빨게 하고 양쪽 젖을 약 6~10분 동안 빨게 한다.
  • 물론 이 때는 이행 젖이나 성숙 젖이 거의 나오지 않는다.
  • 그러나 초유가 한두 방울 정도 나오거나 조금도 나오지 않을 수 있다. 그러나 가능하면 젖을 빨도록 한다.

  • 자연분만으로 태어난 만삭 신생아들의 대부분은 태어나자마자 흡철 반사, 포유 반사등 원시반사가 있고 그런 반사로 바로 엄마의 젖꼭지를 힘차게 잘 빨 수 있다.
  • 하지만 간혹 건강한 아기도 태어난 후 바로 젖꼭지를 빨지 않는 경우도 있다.
  • 그럴 때는 생후 2~4 시간 동안 기다렸다가 다시 젖을 먹이기 시작한다. 분만 후 신생아들은 2~3일 동안은 주로 초유를 먹는다.
  • 요즘 대부분의 산과 병원에서는 수유모와 신생아가 함께 한 입원실에 있을 수 있다. 이처럼 산모와 신생아가 동시에 입원할 수 있는 입원실을 모자동실(母子同室) 또는 모아동실이라고 한다.
